Advance care planning is for people of all ages (so long as you are over 18 and have capacity) and can involve a range of documents, depending on your needs and preferences.
Planning for the future is essential for your peace of mind and that of your friends and family. Advanced care planning empowers you to make informed decisions regarding your healthcare preferences, enabling you to document your decisions and wishes in respect of your medical, well-being and lifestyle choices. Part of this process can also involve you having the opportunity to choose any substitute decision makers (enduring guardians), should this ever be required.
You can also elect to prepare an enduring power of attorney, who is someone that you trust to manage or make decisions in respect of your property and finance if you are ever unable to do so, whether that is temporarily or permanently.
Advance care planning can include a variety of documents, such as enduring powers of guardianship, a values and preferences form, an advance health directive or an enduring power of attorney. By taking the time to clearly document your preferences, choices and to provide some instructions, it can reduce the load on your family or friends, and supports medical teams in providing you with the care you would prefer, wherever possible.
